WHAT IS OSMOLALITY?

Personal lubricants come in a variety of options, from silicone-based and oil-based, to water-based, and hybrids of any and all of these. But depending on their chemical makeup, lubricants can also have varying levels of what scientists call osmolality.

The osmolality rating of a lubricant is important because if osmolality of a lubricant is higher than that of the vaginal mucosa—anywhere above 380mOsm/kg, according to the World Health Organization (WHO)— you risk damaging vaginal cells.

Hyper-Osmotic: If a lubricant has a higher osmolality than the cells of the body it’s called hyper-osmotic. Hyper-osmotic lubes can dehydrate mucus, irritate the skin and cause the epithelial layer of skin to slough off and leave the mucous membrane more susceptible to bacterial infections or STIs. Mucosal irritation has been shown to increase with increasing hyperosmolality. [1]

Hypo-Osmotic: If a lubricant has a lower osmolality than the cells of the body it’s called hypo-osmotic. When tissues are in contact with a hypo-osmotic lubricant they try to pull water from the lubricant into the cell. This means that a slightly hypo-osmotic lubricant would rehydrate dry tissues, but an extremely hypo-osmotic lubricant could cause these cells to burst.

Iso-Osmotic: In a perfect world, every lubricant would be iso-osmotic, meaning it would have equal osmotic pressure, or its cells would have the same hydration levels as human plasma or vaginal fluids, meaning they are perfectly in balance and unlikely to cause any harm. Iso-osmotic lubricant to most of the body’s secretions would have an osmolality range of approximately 250-350 mOsm/kg4.
